I give permission for Glenwood Church to hold and use this information for the purpose of volunteering only. All data will be stored and processed according to GDPR regulations - and will be destroyed once I have withdrawn consent, or at the end of my time volunteering.

I would be willing to provide a reference on request, should one need to be obtained in order to secure my position within the team.

Where the law requires, I am happy to undergo a DBS check to assure my suitability for being a part of the team.
